to Bring to Your Medical

Appointment
letter or other instructions that you received from the U.S.
Government or your legal adviser that show your alien/case
number. Do not bring the email confirmation of your appointment in Montreal, that does not have your case number.

Valid
Photo Identification such as a valid passport or a driver's
license. Children may bring a citizenship certificate or a
birth certificate.

All
your immunization information. See Immunizations to determine
what you need.

Your
payment in cash or money order. We do NOT accept personal
checks, credit or debits cards. See Cost
to determine the amount.

If
you have had a previously abnormal x-ray, blood test, are
taking medication or have a history of serious physical, psychiatric
or psychological problems bring the appropriate information
or documentation with you. See The
Medical for more information.

If
you are pregnant bring a note from your doctor confirming
the pregnancy and indicating the estimated date of delivery.
Please Note: U.S. Regulations currently effective in Canada state: “Women who are pregnant are required to have a chest x-ray to immigrate. Pregnant women will need to provide consent for the chest x-ray.” Our radiologist will provide the lead shielding prescribed to protect your abdominal and pelvic regions. We are unable to exempt anyone from these regulations.
- Three passport photographs
per adults aged 15 and over. One passport photograph for ages 14 and under.
